Available at corporate Starbucks Stores (11-8-07) are these boxes (labeled "created" or "dream") when you purchased a Customized voucher. The white Card inside needs to be activated at the register - then the customer redeems this Card for a personalized Starbucks Card (that you design) at starbucks.com/mycard/ Full instructions are included in the box. This concept of the customized program is discontinued, but custom Starbucks cards can still be made via Starbucks.com.
Again, as of late Summer 2008, this retail concept with the boxes was discontinued from the corporate stores, but it's a great addition to your collection as part of the Starbucks Card program history.
